MOBILE Health Intervention in IntraCerebral Hemorrhage Survivors

This trial is testing whether a mobile health app called WeRISE — which helps survivors of intracerebral hemorrhage (bleeding into the brain) monitor and manage their blood pressure at home — can improve outcomes and reduce the risk of another stroke or brain bleed. **You may be eligible if...** - You are 18 years or older - You have been diagnosed with intracerebral hemorrhage (bleeding into the brain, not from trauma) - At hospital discharge, your level of disability (Modified Rankin Scale) is 4 or less (meaning you have some ability to function independently or with minimal assistance) - You or your caregiver have access to and can use the WeRISE app and can monitor blood pressure at home **You may NOT be eligible if...** - Your life expectancy is less than 1 year - You are unable to use the app or participate in follow-up - You cannot safely lower your blood pressure (e.g., you have severe artery narrowing in the brain, a history of fainting from low blood pressure, or other contraindications) - You have a severe bleeding disorder or very low platelet count - You have severe kidney or liver disease - You are allergic to multiple classes of blood pressure medication Talk to your doctor to see if this trial is right for you.
