This study looks at whether the position a child is placed in during a lumbar puncture (spinal tap) affects how much pain they experience afterward, particularly the type of headache that can sometimes follow this procedure. The study involves children who need lumbar punctures as part of their treatment for cancer. You may be eligible if: - Your child is between 4 and 15 years old - Your child is receiving treatment for a cancer and needs a lumbar puncture as part of their standard care - The procedure requires general anaesthesia and will be done in day surgery You may NOT be eligible if: - Your family needs an interpreter - Your child is having a bone marrow procedure on the same day - A specially trained doctor is not available - Your child needs an emergency lumbar puncture outside the day surgery unit - Your child already has a headache caused by a previous lumbar puncture Talk to your doctor about whether this trial might be right for you.

Interventions

This is a double blind, randomised crossover trial. A single experienced doctor will perform all of the lumbar punctures (LP) under general anaesthesia. A standard LP needle (PORTLAND) will be used. O

This is a double blind, randomised crossover trial. A single experienced doctor will perform all of the lumbar punctures (LP) under general anaesthesia. A standard LP needle (PORTLAND) will be used. Orientation of the bevel will be parallel to the long axis of the dural fibres. The doctor cannot be blinded to the needle type. All children will be given a sequence of 4 LP's during their treatment for cancer. Twice with each of the 22g and 25g LP needles. Following each procedure, parents will be given a 1 page questionnaire by the RA. The same RA will phone interview them on days 1, 3, and 7 with the same questions. On day 7 more detailed questions will be asked if the patient experiences any headache. These questions will look into issues such as, timing of headache, analgesia required and functional impact on the family.
