This study aims to implement a clinical transition pathway utilising a collaborative, shared-care model between The Royal Children's Hospital Melbourne (RCH) and Peter MacCallum Cancer Centre (PMCC). Who is it for? You may be eligible to join this study if you are aged 16 years and older, and have a cancer diagnosis or a condition cared for under the RCH Children’s Cancer Centre which is your main medical issue. You need to have finished treatment within the last 2.5 years or be on indefinite, maintenance type treatment. PMCC must be a suitable place to transition to for your ongoing care. Study details All participants who meet the eligibility criteria in this study will begin a clinical pathway for transition from paediatric to adult services for AYA diagnosed with cancer. It will comprise four time points over 3-6 months: Baseline - Orientation and Consent, time points 1, 2, and optional 3 - Transition shared care clinic model. The program will be evaluated throughout intervention period and participants will be assessed for experience of care, client satisfaction, transition readiness, information needs. It is hoped that this research result will help develop and deliver a sustainable model of care that improves the experience of transition for young people with cancer and the skills and systems around transition for adult and paediatric cancer teams.

Implementation of a clinical pathway for transition from paediatric to adult services for AYA diagnosed with cancer. The total duration will be 4-9 months. It will comprise four time points: Baseline: Orientation and Consent (RCH) over 1-3 months • Referral to AYA-PACT clinic • Orientation to the transition program and consent Time points 1, 2 and optional 3 : Transition shared care model (over 3-6 months) • Paediatric cancer care continues - Royal Children's Hospital (RCH) • Adolescent Young Adult Paediatric Adult Collaborative Transition (AYA-PACT) multidisciplinary clinics every 3 months - Peter MacCallum Cancer Centre (PMCC). Consented patients will attend 2 or 3 AYA-PACT one hour clinics, depending on individual support and transition preparation needs. Screening measures will be completed to guide and personalise content prior to each clinic. Clinics will address: - transition preparation and orientation to adult hospital areas - supports such as education, social work, psychology, exercise physiology - referrals as required, - symptom management according to screening measures - informational needs with resource provision - including understanding of diagnosis and treatment, potential for future late effects, health and wellbeing after cancer treatment. The clinics will be led by an AYA oncologist, AYA clinical nurse consultant, AYA oncology fellow and involve referrals and support delivered through the multidisciplinary AYA team as required. The clinic will be held in the Youth Cancer Centre at PMCC. The first clinic will be face to face with option for telehealth at subsequent clinics. Resources developed by young people for transition and survivorship will be utilised during the clinic including a transition video. An on-line survivorship resource developed by the AYA service with input from young people, including information, videos and podcasts about life after treatment will also be used. Medical handover from paediatric to adult cancer services will occur during the intervention period. Evaluation utilises implementation science framework and includes mixed methods quantitative data, surveys, interviews
