Mechanisms of Pregnancy Vascular Adaptations

Angiotensin 2 Receptor (AT2R) Expression/Activation in Endothelial Cells in Preeclampsia

Sponsor

University of Wisconsin, Madison

Enrollment

166 participants

Start Date

Nov 20, 2018

Study Type

OBSERVATIONAL

Conditions

Summary

The investigators will collect omental tissue (research surgical excision) and placental tissue (standard of care clinical delivery) from both preeclamptic and non-preeclamptic women during their c-section and use these samples to study the blood vessels, specifically the expression/activation of the AT2R.

Plain Language Summary

Simplified for easier understanding

This study is investigating how blood vessels in the body adapt during pregnancy — specifically, how and why some pregnant women develop high blood pressure (preeclampsia) while others do not. **You may be eligible if...** - You are a woman between 18 and 40 years old - You are pregnant with a single baby between 28 and 41 weeks along - You are scheduled for a cesarean section (planned or emergency) **You may NOT be eligible if...** - You are currently being treated for severe high blood pressure with medications or magnesium sulfate after a preeclampsia diagnosis - Your baby has a major structural problem or chromosomal condition - Your cesarean section is being performed because of a placental tear or major bleeding - You are not between 36 and 41 weeks of pregnancy at the time of enrollment Talk to your doctor to see if this trial is right for you.

Interventions

PROCEDUREOmental Biopsy

The surgeon and clinical care team will conduct the c-section to deliver the baby and the placenta as clinically indicated. After delivery, during the closing procedures, the surgeon will indicate a time for a participating Ob/Gyn physician on the study team to collect the omental bio-specimen. Omentum is frequently excised or removed for clinical reasons during surgical procedures. Omentum has small blood vessels within it that have been used for evaluation of vascular functional changes in research. The investigators wish to isolate one to three vessels of 5 to 7 cm length. The surgeon will then close and the clinical care team will resume their standard of care activities.
