Summary

The goal of this research is to increase patients' knowledge about advance care planning (ACP) discussions and hospice care for patients with serious illness receiving home-based care.

Eligibility

Min Age: 21 Years

Plain Language Summary

Simplified for easier understanding

This study is testing whether showing a short educational video about advance care planning (ACP) and hospice care to seriously ill patients receiving home-based medical care helps them better understand their options and have clearer conversations with their doctors about their wishes. ACP is the process of deciding in advance what kind of medical care you would want if you were too ill to speak for yourself. Adults aged 21 and older with a serious illness receiving home care through Clover Health's House Calls Program, who have a life expectancy of one year or less and speak English or one of 25 other available languages, are eligible. Participation involves being randomly assigned to either watch the video (in addition to usual care) or continue with usual care alone, with surveys to measure knowledge and communication afterward. This summary was prepared with AI assistance to help patients understand the study in plain language.

This summary was AI-generated to explain the trial in plain language. It is not medical advice. Always discuss eligibility with your doctor before enrolling in a clinical trial.
