Multidimensional Approach for COPD and High Complexity

This study is enrolling people with COPD (chronic obstructive lung disease) or heart failure who also have at least two other chronic conditions, in order to test and evaluate a comprehensive multidimensional care approach for complex, multi-disease patients. A healthy control group is also included for comparison. **You may be eligible if...** - You have a confirmed diagnosis of COPD or heart failure - You also have at least two other chronic conditions (such as diabetes, high blood pressure, kidney disease, anemia, liver disease, or obesity) - You are able and willing to comply with study visits and procedures - You are a healthy adult (for the control group) **You may NOT be eligible if...** - You are an active smoker - You need continuous oxygen therapy - You can walk less than 300 meters in 6 minutes - You have unstable angina or recent hospitalization for heart failure or COPD Talk to your doctor to see if this trial is right for you.
