This study is testing whether an AI-guided echocardiogram (ultrasound of the heart) performed in a primary care setting can help identify patients with undiagnosed heart failure or heart valve disease earlier, potentially preventing hospitalizations. **You may be eligible if:** - You are over 45 years old - You are covered by Medicare - You have exercise intolerance or cardiovascular risk factors (such as high blood pressure, obesity, or diabetes) **You may NOT be eligible if:** - You have already been diagnosed with heart failure or heart valve disease - You already have an established indication for heart-protective treatment (such as known coronary artery disease) - You have a serious illness with a life expectancy of less than 2 years - You are unable to provide written informed consent Talk to your doctor to see if this trial is right for you.
