Summary

This study plans to learn more about heart function among individuals with chronic obstructive pulmonary disease (COPD). In particular, the investigators want to understand the different patterns of right ventricular response to pulmonary hypertension (high pressure in the lungs) during rest and moderate- vs high-intensity exercise. By identifying patterns of right ventricular dysfunction, this study will help identify better treatments for patients with COPD in the future.

This study is looking at how COPD (a lung disease) affects the right side of the heart. Researchers will use heart catheterization, imaging, and exercise tests to better understand right heart problems in COPD patients whose lung scans show an enlarged pulmonary artery. **You may be eligible if...** - You have been diagnosed with COPD confirmed by breathing tests - Your chest CT scan shows an enlarged pulmonary artery (the vessel from the heart to the lungs) **You may NOT be eligible if...** - You had a COPD flare-up or took oral steroids in the past 3 months - You changed your COPD medications in the past 3 weeks - You need high-flow supplemental oxygen - You have uncontrolled high blood pressure, heart failure, significant heart valve disease, or poorly controlled diabetes - You have another lung disease such as pulmonary fibrosis - You have significant fluid retention (swelling) Talk to your doctor to see if this trial is right for you.

Interventions

BEHAVIORALExercise training

Exercise training will include moderate-intensity interval training for \~35 minutes 3x/week and light-intensity continuous aerobic exercise for \~25 minutes 2x/week.
