Summary

The goal of this clinical trial is to analyse the prevalence of new or uncontrolled arterial hypertension (AHT) after hospital discharge of medical in-patients with elevated blood pressure (BP) values during hospitalisation. The main questions it aims to answer are: * Do elevated BP values during hospitalisation correspond to new or uncontrolled AHT after hospital discharge? * Is it safe to postpone adaption of antihypertensive treatment until after proper evaluation of AHT after hospital discharge? Participants will either be treated according to their physicians' decision or antihypertensive treatment adaptions will be postponed until after hospital discharge.

Plain Language Summary

Simplified for easier understanding

This study looks at how to best identify and manage elevated blood pressure that is discovered unexpectedly while a patient is in the hospital for a different (non-heart-related) reason. Researchers will use portable blood pressure monitoring to understand whether hospital readings reflect true high blood pressure or a temporary reaction to being in the hospital. **You may be eligible if...** - You are hospitalized on a medical ward for a non-heart-related condition - Your blood pressure has been elevated (140-180 systolic or 90-110 diastolic) on at least 2 separate readings without causing symptoms - You are able to understand the study procedures and provide written consent **You may NOT be eligible if...** - You were hospitalized for a stroke, heart attack, acute heart failure, or other condition worsened by high blood pressure - You are pregnant or breastfeeding - Any condition prevents you from wearing a blood pressure monitor Talk to your doctor to see if this trial is right for you.
