This study validates a structured management process for treating anemia (low red blood cell count) in people who are on long-term kidney dialysis (hemodialysis), to improve blood count management and reduce complications related to renal anemia. **You may be eligible if...** - You are 18 or older - You have been receiving regular hemodialysis at least twice a week for at least 3 months - You plan to continue dialysis for at least 6 more months - You are willing to participate **You may NOT be eligible if...** - You are planning a kidney transplant or expecting to stop dialysis during the study - You have a bone marrow disease such as myelodysplastic syndrome or multiple myeloma - You have a hereditary blood disease such as thalassemia, sickle cell anemia, or red cell aplasia - You have active or chronic gastrointestinal bleeding - You had a serious infection within the past week Talk to your doctor to see if this trial is right for you.
