This Phase II study is testing whether a community health worker program — using remote blood pressure monitoring and culturally tailored support — can help Latino patients with uncontrolled high blood pressure (hypertension) better manage their condition and take their medications regularly. **You may be eligible if...** - You self-identify as Latino/Latinx - You are 18 or older - You speak English or Spanish - You receive care at one of the 10 participating safety-net clinics - Your blood pressure has been uncontrolled (average 140/90 mmHg or higher) on at least two clinic visits in the past year - You have been prescribed at least one blood pressure medication but have been taking it less than 80% of the time **You may NOT be eligible if...** - You cannot follow the study protocol - You are currently in another hypertension-related clinical trial - You have significant psychiatric issues or substance abuse documented in your medical record - You plan to stop receiving care at the clinic within the next 15 months Talk to your doctor to see if this trial is right for you.
