Targeted Palliative Care Intervention for Patients With Metastatic Breast Cancer

This study tests a targeted palliative care program — focused on comfort, quality of life, and planning for the future — for patients with metastatic breast cancer (cancer that has spread beyond the breast). A support program will be offered alongside regular cancer treatment, and the study will also support caregivers. **You may be eligible if...** - You are 18 or older and have been diagnosed with metastatic breast cancer - You were recently identified as potentially needing end-of-life care planning - You are functioning reasonably well and can complete questionnaires in English or Spanish - You receive your cancer care at one of the participating hospitals - You have not had an outpatient palliative care visit in the last 6 months **You may NOT be eligible if...** - You are already enrolled in hospice care - You have a condition that prevents you from understanding or consenting to the study - You already received outpatient palliative care in the past 6 months Talk to your doctor to see if this trial is right for you.
