This study is testing a graduated care support program (GRACE) for people with advanced COPD who have had at least one flare-up in the past year — delivering stepped levels of support based on individual need to improve quality of life and symptom management. **You may be eligible if...** - You have COPD (chronic obstructive pulmonary disease) - You have had at least one exacerbation (flare-up) in the past year - You live in the Høje Taastrup or Copenhagen area of Denmark - You are cognitively able to participate **You may NOT be eligible if...** - You have dementia - You have psychosis or active substance abuse - You have hearing or speech impairments that prevent participation - You have cancer or are already receiving specialized palliative care - You are in the final stages of life (immediately dying) Talk to your doctor to see if this trial is right for you.
