This trial is comparing two approaches to managing poorly controlled type 2 diabetes: working with a registered dietitian-nutritionist versus attending a specialized diabetes day hospital program. **You may be eligible if...** - You have type 2 diabetes with poor blood sugar control - You have been referred to an endocrinology specialist for this reason - You are currently using insulin therapy **You may NOT be eligible if...** - You have a type of diabetes other than type 2 (e.g., type 1 or gestational) - You use fast-acting insulin bolus therapy (e.g., a mealtime insulin regimen) - You are referred for reasons other than poor blood sugar control (e.g., low blood sugar episodes) - You are having an acute diabetes emergency - You are taking corticosteroids - You have significant cognitive impairment or require help with basic daily activities (Barthel index below 60) - There is a language barrier that prevents participation Talk to your doctor to see if this trial is right for you.
