This study is testing a web-based tool called 'My Voice' to help kidney failure patients on dialysis plan ahead for their medical care, including end-of-life decisions. The tool is designed to make it easier for patients and their families to have these important conversations. **You may be eligible if...** - You are a Singapore citizen or permanent resident aged 21 or older - You have severe chronic kidney disease (stage 5) and are currently on dialysis - Your health team has identified you as someone who may benefit from serious illness conversations (for example, your condition has been getting worse, dialysis has become hard to tolerate, you've been hospitalised multiple times recently, or you have limited ability to do daily tasks) - You have normal cognitive function (no significant memory or thinking problems) - You can speak and read English, Mandarin, or Malay **You may NOT be eligible if...** - You have cognitive impairment or a serious mental illness - You have already had formal serious illness conversations with your care team - You are not easily reachable by phone Talk to your doctor to see if this trial is right for you.
