This study is testing whether a structured physical rehabilitation program during and after a hospital stay can improve recovery in older adults who are admitted for worsening heart failure with preserved ejection fraction (a type of heart failure where the heart squeezes normally but is too stiff). **You may be eligible if...** - You are 60 years or older - You have been hospitalized for more than 24 hours for worsening heart failure - Your heart's squeezing function (ejection fraction) is 45% or higher - You have at least one worsening symptom (like shortness of breath or fatigue) plus at least two signs of fluid buildup confirmed by your doctors - Your treatment has been adjusted specifically to address the heart failure episode **You may NOT be eligible if...** - You do not meet the above criteria for confirmed acute decompensated heart failure - You are below 60 years of age - You have a different type of heart failure (reduced ejection fraction below 45%) Talk to your doctor to see if this trial is right for you.
