Diabetes Toolkit at Discharge

This study evaluates a structured diabetes education program (called the Diabetes Toolkit) given to patients at hospital discharge when they are newly prescribed insulin. The goal is to help patients safely manage insulin at home and reduce the risk of readmission due to blood sugar problems. **You may be eligible if...** - You are 18 or older - You speak English - You have been diagnosed with diabetes - You are being discharged from the hospital with a new insulin prescription (insulin you have not used before) - Your life expectancy is more than 90 days **You may NOT be eligible if...** - You do not speak English - You are unable to give informed consent - You have not been diagnosed with diabetes - You are not being newly prescribed insulin at discharge Talk to your doctor to see if this trial is right for you.
