Prognostic Model Heart Failure

This study is building a tool to predict how well patients with heart failure will do over the next 30 days after visiting an emergency department. Researchers are collecting information from patients to create a risk model that can help doctors identify who needs closer follow-up care. **You may be eligible if...** - You are 18 or older - You are visiting or have visited the emergency department at NICVD in Karachi, Pakistan - You have been newly or previously diagnosed with heart failure - You are willing and able to be contacted by phone or in-person for a follow-up 30 days later - You are willing to give informed consent **You may NOT be eligible if...** - You are under 18 years old - You do not consent to participate - You have a severe neurological or psychiatric condition (other than depression or anxiety) that affects your ability to participate Talk to your doctor to see if this trial is right for you.
