Improving PCP Advance Care Planning for People With ADRD

This study is testing a toolkit called the AD ACP Toolkit — a structured system to help primary care doctors and their teams have better advance care planning conversations (about future medical wishes) with patients who have Alzheimer's disease or related dementia (ADRD). The goal is to improve the quality of end-of-life planning for these patients. **You may be eligible if...** - You are a primary care provider (doctor, nurse practitioner, or physician assistant) who sees older patients with dementia - You are a patient aged 65 or older with a confirmed diagnosis of Alzheimer's disease or a related dementia - You are seen at a participating UNC Health primary care clinic **You may NOT be eligible if...** - You are a provider who does not care for older adults (e.g., pediatrics) - You are a patient without a confirmed dementia diagnosis - You have not had a primary care visit in the past 18 months Talk to your doctor to see if this trial is right for you.
