This study tests a combination approach for people with liver cancer (hepatocellular carcinoma, or HCC) that has spread into the main portal vein — the major blood vessel supplying the liver. Treatment involves a radioactive stent to reopen the vein, followed by a liver artery procedure (TACE) plus immunotherapy and targeted drugs. **You may be eligible if...** - You are 18 or older with confirmed liver cancer - Your cancer has spread into the main portal vein (called Vp4 portal vein tumor thrombus) - You have at least one measurable tumor in the liver - You are in acceptable health (ECOG ≤ 2) - You have received first-line treatment with this combined approach within 8 weeks **You may NOT be eligible if...** - Your cancer has spread to other organs outside the liver - You have had other primary cancers - You received prior locoregional or systemic treatments within 3 months before this stent procedure - Your liver function is very poor (Child-Pugh grade C) - Your records are incomplete Talk to your doctor to see if this trial is right for you.
