UNCPM 22322 - Adaption of the Transition of Care Model for Post-Discharge HIV-NCD Care in Lilongwe, Malawi - MLATHO

This study in Malawi adapts a care transition model to help HIV-positive patients who are also dealing with heart disease, diabetes, high blood pressure, or other long-term conditions after being discharged from the hospital. The goal is to improve follow-up care for both HIV and these other chronic conditions. **You may be eligible if...** - You are 18 years or older - You are living with HIV - You were admitted to an internal medicine ward - You have at least one cardiometabolic condition (such as high blood pressure, diabetes, or heart disease) that contributed to your hospital admission **You may NOT be eligible if...** - You live outside of Lilongwe urban area (as follow-up visits would be impractical) Talk to your doctor to see if this trial is right for you.
