**You may be eligible if...**
- You have been implanted with, or are scheduled to receive, a CCM device made by Impulse Dynamics for heart failure
- You are willing and able to provide informed consent, including consent for your data to be used in research and publications
- You are committed to attending follow-up visits and taking your medications as directed
**You may NOT be eligible if...**
- Your doctors believe you are unlikely to follow through with follow-up appointments or medication adherence
- You have a mechanical tricuspid valve (an artificial replacement of the heart's tricuspid valve)
