This trial is testing whether providing medically tailored food boxes (food as medicine) to veterans with moderate to advanced chronic kidney disease (CKD) who also face food insecurity can improve their nutrition and slow disease progression. **You may be eligible if...** - You are 18 years of age or older - You have been diagnosed with CKD stage 3b or 4 - You screen positive for nutrition insecurity (difficulty affording or accessing nutritious food) - You receive care at the VA and live in the Food Bank of the Rockies service area - You can speak English and provide informed consent **You may NOT be eligible if...** - You have early-stage CKD (stages 1–3a) or end-stage kidney disease (stage 5, on dialysis, or post-transplant) - You are currently enrolled in a conflicting dietary study - You are unable or unwilling to provide informed consent Talk to your doctor to see if this trial is right for you.
