This study is testing whether a brain stimulation device called deep brain stimulation (DBS) — where a small implanted device sends electrical pulses to specific areas of the brain — can more precisely target the right areas to help people with severe, treatment-resistant obsessive-compulsive disorder (OCD). **You may be eligible if...** - You are between 20 and 65 years old - You have been formally diagnosed with OCD - You have had OCD for at least 5 years - You have tried and failed multiple medications for OCD - You have tried and failed cognitive behavioral therapy (talk therapy) for OCD - Your OCD symptoms are rated as very severe (scored at least 30 out of 40 on a standard OCD scale) **You may NOT be eligible if...** - You have a severe personality disorder (such as borderline or antisocial personality disorder) - You have significant brain abnormalities on an MRI scan - You have ever experienced psychosis (hallucinations or delusions) - You currently use drugs or alcohol, or have done so within the past 12 months - You have any other condition affecting thinking or memory - You have significant intellectual disability Talk to your doctor to see if this trial is right for you.
