This trial tests whether implanting a small heart monitoring device to measure blood pressure inside the heart can help doctors adjust medications faster and prevent serious complications in patients who have recovered from a cardiogenic shock (a life-threatening event where the heart suddenly stops pumping well enough). **You may be eligible if...** - You are 18 or older - You have been hospitalized with cardiogenic shock and survived to discharge - You have heart failure with significant breathlessness even with mild activity (NYHA class III) - You have internet and phone access (needed for the device to send data) **You may NOT be eligible if...** - You are already using a long-term mechanical heart pump (LVAD) or have had a heart transplant - Your doctors believe the procedure is too risky for you - You cannot follow up or attend required appointments Talk to your doctor to see if this trial is right for you.
