It aims to see whether digital tools can help people better control their blood pressure and reduce the risk of heart attacks and strokes. **You may be eligible if...** - You are between 30 and 90 years old - You own a smartphone - Your blood pressure measured in a clinic is 140 mmHg or higher (systolic) - You are currently taking 2 or fewer blood pressure medications - You speak English or Spanish - There are at least 2 medication types available for your doctor to add or adjust **You may NOT be eligible if...** - You have severe heart failure (ejection fraction below 40%) - Your kidneys are severely failing or you are on dialysis - You are pregnant - You have had a heart attack or stroke in the past 6 months - You have had a hypertensive emergency requiring hospitalization in the past 6 months - You have had a solid organ transplant Talk to your doctor to see if this trial is right for you.
