This study tests whether combining a digital health program (apps or wearables for home monitoring) with a pulmonary rehabilitation program improves breathing and quality of life for people with moderate to severe COPD (chronic obstructive pulmonary disease — a condition where the airways are damaged, making it hard to breathe). **You may be eligible if...** - You are between 55 and 65 years old with COPD (any gender) - Your COPD is moderate to severe (breathing test shows FEV1 below 80% of expected) - You have emphysema visible on a CT scan - You have class II obesity (BMI between 35–40) - You have a smoking history of more than 10 pack-years **You may NOT be eligible if...** - Your COPD is very severe, or you have lung cancer or lung fibrosis - You have heart failure - You have recently had a COPD flare-up or respiratory infection Talk to your doctor to see if this trial is right for you.
