This study (EVOLAIRE) combines genetic risk scoring (a polygenic risk score calculated from your DNA) with CT heart imaging to help doctors better guide how aggressively to manage cardiovascular risk factors in people with Type 2 diabetes. The goal is to personalize heart disease prevention. **You may be eligible if...** - You are 40 or older - You have a confirmed diagnosis of Type 2 diabetes - Your CT scan imaging quality is acceptable - A polygenic risk score can be calculated from your genetic data **You may NOT be eligible if...** - You already have known cardiovascular disease (e.g., prior heart attack or stroke) - Your CT scan shows significant coronary disease that already requires intervention - You are unable or unwilling to provide informed consent or attend follow-up visits Talk to your doctor to see if this trial is right for you.
