Facilitated Transitions From Postpartum to Primary Care Coordination for People With Chronic Conditions

This study is testing a program to help pregnant women or new mothers with chronic health conditions (like high blood pressure, diabetes, or depression) transition smoothly from obstetric care to ongoing primary care after having a baby. It uses the patient portal and care coordination support. **You may be eligible if...** - You are currently pregnant or recently gave birth (at least 24 weeks) at an MGH-affiliated obstetrics clinic - You have at least one of the following: high blood pressure (including preeclampsia), diabetes (type 1, type 2, or gestational), obesity (BMI ≥ 35), or depression/anxiety - You have a primary care doctor listed in your medical records - You can read and speak English or Spanish **You may NOT be eligible if...** - You are receiving care at the MGH HOPE Clinic (which already provides long-term primary care) - You delivered before 24 weeks gestation - You do not have a primary care provider on record - You are unable to use the electronic patient portal Talk to your doctor to see if this trial is right for you.
