This study is evaluating whether a preventive, garden-based group therapy program — run through primary care practices — is acceptable and beneficial to patients with heart and blood vessel diseases such as stroke, heart failure, arrhythmia, or coronary artery disease. **You may be eligible if...** - You are older than 18 years - You live within approximately 20 km of Gap, Laragne-Montéglin, Veynes, or Clermont-Ferrand in France - You have been invited to participate by your care team - You have a cardiovascular condition covered by the French national health criteria (such as stroke, peripheral artery disease, heart rhythm disorder, valvular disease, congenital heart disease, stage I–II heart failure, severe hypertension, or chronic coronary disease) **You may NOT be eligible if...** - You have advanced heart failure (NYHA class III or IV) - You have a contraindication to physical activity - You live in a care institution - You cannot walk more than 100 meters independently - You have Alzheimer's disease or significant cognitive impairment Talk to your doctor to see if this trial is right for you.
