Medically Intensive Nutrition Therapy Program for Obesity and Diabetes in a Low-income Population

This study is testing a medically intensive nutrition therapy program aimed at improving blood sugar control and reducing weight in low-income patients with obesity and diabetes or pre-diabetes. **You may be eligible if...** - You are between 18 and 65 years old - Your BMI is 27 or higher - You have an HbA1c of 5.7% or higher, or a fasting blood sugar of 100 mg/dL or higher (indicating diabetes or pre-diabetes) - Your family income is at or below 400% of the federal poverty level **You may NOT be eligible if...** - You currently take diabetes medications other than metformin, or approved weight-loss medications - Your HbA1c is 10% or higher (very poorly controlled diabetes) - You are planning bariatric (weight loss) surgery - Other health conditions or plans apply (your doctor will review) Talk to your doctor to see if this trial is right for you.
